Help Me Grow specialists provide screenings for children birth to 3 years of age to identify and overcome developmental delays and provide service coordination for parents.  Parent education is also available through the Early Head Start program.  For more information call 937-642-2053.

Ages and Stages Questionnaire-3 (ASQ-3)

Ages and Stages Questionnaire: Social Emotional (ASQ-SE)

The Union County Health Department is providing Union County residents with a free and easy way to screen your child to determine if they are developmentally “on track”. The Ages and Stages Questionnaire -3 (ASQ-3) and the Ages and Stages Questionnaire: Social Emotional (ASQ-SE) screening tools can help your family determine if your child is reaching their developmental milestones. 

The ASQ-3 and the ASQ-SE are nationally recognized reliable screening tool for children ages birth to five. These screening tools will help your family to determine if you may want to pursue further evaluation of your child’s development.  

The ASQ-3 is a screening tool for your child’s fine motor, gross motor, and speech. To use the ASQ-3 to screen your child, click here: ASQ-3

The ASQ-Social Emotional screening tool screens your child on attachment, interaction with family and friends, and ability to adjust to their environment. To use the ASQ-SE to screen your child, click here: ASQ-SE

Once you complete the screening tools the results will be sent to the Union County Health Department. All information that you provide including your contact information and developmental information will be treated as medical information and will not be shared with anyone without your written consent. 

Within five business days of completing the screening a child development professional will contact you either by phone or mail with the screening results.
