This clinic will serve:

• Adults ages 25 through 64 years with chronic medical conditions such as asthma, diabetes, heart disease, morbid obesity, etc. 
• Heathcare workers and EMS with direct patient contact
• Pregnant women (Must have letter approving H1N1 vaccination from their doctor) 

Tickets will be handed out no earlier than 10:30 am for vaccinations from 11 am-1:30 pm. Tickets will be handed out no earlier than 2:30 pm for vaccinations from 3-7pm. Tickets will be handed out until 7 pm or vaccine is depleted. Please have only one family member pick up tickets for all eligible family members.

Please pre-register online at www.flu.ohio.gov and print out and bring your registration form to the clinic.
