This clinic will serve:

• Children ages 6 months through 24 years
• Caregivers of infants under age 6 months (includes parents, siblings, and daycare providers)

Tickets will be handed out no earlier than 2:00 pm and will be distributed until 7 pm or vaccine is depleted. Vaccinations will be from 3-7pm. Please have only one family member pick up tickets for all eligible family members.

Please pre-register online at www.flu.ohio.gov and print out and bring your registration form to the clinic.
