This H1N1 Vaccination clinic is for:

Pregnant Women- must bring a letter from your doctor confirming pregnancy
EMS with Direct Patient Care- no ID required
All Healthcare Workers with Direct Patient Care (Including those at doctor's offices, long-term care facilities, home health agencies, dental offices, etc.)- no ID required
Daycare Center Staff in Contact with Infants Under Age 6 Months (This does not include in-home daycare providers as these providers and their family members in contact with infants under age 6 months will be vaccinated next week)- no ID required

Vaccination is free. All clinics are based on availability of vaccine and may be cancelled or closed early if vaccine supplies are depleted.
